Outpatient Diabetes Education
Our Diabetes Education Program helps people with type 1 and type 2 diabetes live healthy, productive lives. The program is certified by the American Diabetes association and is taught by masters prepared staff with additional certification in diabetes education.
Once your physician has sent a referral to us, we will contact you to schedule your first visit. The program begins with an individual visit with our registered dietitian. She will complete an assessment to determine your educational needs and develop a meal plan with you at that time. You will then be scheduled for 2 group classes which will cover the following topics:
General facts of diabetes Medication
Nutrition Sick day care
Monitoring blood sugars Complications of diabetes
High and low blood sugars Goal setting/ problem solving
Diabetes resources Behavior change
Exercise
We also have a gestational /diabetes and pregnancy education program which is taught individually and in 1 session. An educational assessment is completed; nutrition instruction is provided and a meal plan is developed with you by our registered dietitian. You then see the RN for instruction regarding:
General facts of gestational diabetes or diabetes and pregnancy facts
High/low blood sugars Sick days care
Exercise Monitoring blood sugars
Medications Goal setting/problem solving
Urine ketone monitoring
