Community Health Needs Assessment

Under the direction of the Granville Vance District Health Department (GVDHD), a comprehensive Community Health Needs Assessment has been devised for the two counties in the Health Department’s district, which includes Granville and Vance Counties. The CHNA serves as a reference for selection of strategic issues for Maria Parham Health.   

It is the goal of the CHNA to describe the health status of the population, identify areas for health improvement and, determine factors that contribute to health issues, and identify assets and resources that can be mobilized to address public health improvement.   This assessment will be updated and revised periodically, thus providing communities with up-to-date data to evaluate progress made towards identified health priorities, and for the selection of new ones.
